Hospice Medical Director
The hospice physician plays a vital role in guiding and supporting the hospice team. This doctor actively participates in hospice meetings and collaborates with your primary physician to ensure that your medical care is consistently monitored and tailored to your ongoing needs. They are responsible for conducting a “Face-to-Face” visit and making house calls as necessary to provide personalized care.
Attending Physician
Your doctor will continue to be your primary physician unless you decide to make a change. Your nurse will maintain open communication with your doctor to keep them informed about any changes in your condition or needs. Your physician will still prescribe your medications and will oversee any adjustments to your care plan. As part of the interdisciplinary team, your attending physician will actively contribute to creating a comprehensive assessment tool that emphasizes clinically relevant information.
Registered Nurse (RN)
Upon admission to IDT Hospice Care, Inc., you will be assigned a registered nurse (RN) who will provide personalized visits, support, and education for you and your caregiver(s). This nurse will coordinate your care throughout your time with us, ensuring that your physician is kept informed about your condition. Scheduled visits will focus on monitoring your terminal illness, managing pain, and addressing any symptoms you may experience, with the frequency of visits adjusted based on your individual needs.
On-Call Nurse
A nurse is available 24 hours a day to answer any questions you may have. If needed, an on-call nurse will visit you to resolve any concerns that may occur during nights or weekends.
Hospice Aides
Hospice aides provide essential personal care to patients in their homes, assisting with daily activities such as bathing, grooming, and feeding. Their compassionate support helps ensure comfort and dignity during end-of-life care. Aides work under the supervision of nurses, creating a holistic approach to patient well-being.
Social Workers
Social workers at IDT Hospice Care are here to help you navigate the emotional challenges that come with serious illness, providing support to you and your caregivers whenever needed. They assist with healthcare decisions, funeral arrangements, and legal matters, connecting you with essential community resources. Their key responsibilities include conducting psychosocial assessments, coordinating care, offering counseling and psychotherapy, intervening during crises, and educating clients and families about available resources and support systems.
Chaplain
Our non-denominational chaplains offer compassionate spiritual care tailored to your needs. Upon request, they can help explore spiritual concerns such as fear, loss, forgiveness, or anger and address any questions you may have. Additionally, they can facilitate connections with your local church to arrange visits from your priest, rabbi, minister, or spiritual advisor.
Bereavement Counselors
At IDT Hospice Care, Inc., our bereavement counselors offer compassionate, ongoing support to caregivers and family members navigating their grief. Through phone calls, mailings, and support groups, they provide encouragement and comfort, ensuring that families receive care for up to 13 months following the patient’s passing.
Hospice Volunteers
Our volunteer services focus on providing companionship to patients and their loved ones, offering empathy and support during challenging times. Volunteers engage in various activities, such as meal preparation, grocery shopping, playing music, and enjoying movies or games together. Additionally, volunteers may assist with light office tasks, providing essential administrative support to our agency and ensuring smooth operations.
