IDT Hospice Care services offer comprehensive support, including:
- Personal care for patients, such as bathing and feeding
- Assistance with household chores to ease daily burdens
- Transportation to and from the inpatient unit for convenience
- Supplies, equipment, and medications related to the primary illness
- Emotional support and counseling for both patients and families
- Spiritual counseling to address deeper concerns
- Short-term care for issues that cannot be managed at home
- Follow-up bereavement counseling for family members after the patient’s passing
We also contract with nursing facilities for short-term inpatient care and offer continuous care for patients with uncontrolled symptoms. Upon admission, the admitting RN creates an individual plan of care, and the IDT team is dedicated to meeting the patient’s goals with dignity and support.
