Hospice Level of Care

IDT Hospice Care, Inc. offers flexible shifts of hospice staff to assist patients in their homes during brief periods of need. If a patient’s care requirements exceed what can be safely managed at home, we have established partnerships with local hospitals and skilled nursing facilities. This ensures that patients receive the necessary inpatient care to address any urgent situations until they can safely return home.

Hospice Levels of Care:

• Routine Home Care
  Experience compassionate hospice care right in the comfort of your home with our routine home care services. Routine Home Care is the most common level of care for hospice patients. Our dedicated team monitors each patient’s status during visits, ensuring that the care plan is continuously updated to address the evolving needs of both patients and families.
• Respite Care
  Respite care offers essential short-term relief for in-home hospice caregivers. This service allows family caregivers to take a much-needed break while ensuring that their loved ones receive compassionate care. IDT Hospice Care partners with designated facilities to provide high-quality respite services. During this time, patients are transferred to a contracted facility for a temporary stay, typically not exceeding five days. This arrangement helps caregivers recharge while maintaining the comfort and safety of the patient.
• Continuous Care
  Our dedicated team is available to provide 24/7 care and support through our continuous care services. This level of care begins when a patient experiences symptoms requiring ongoing monitoring for effective management. A nurse or aide will be present at the bedside for at least 8 hours within a 24-hour period, with the possibility of up to 24 hours of care each day. This short-term care is designed to stabilize symptoms and ensure the patient’s comfort. Our goal is to help manage symptoms effectively and provide relief during challenging times.
• Inpatient Care
  When symptom management requires more specialized care, our inpatient care services are here to provide comfort and support. Inpatient care is offered in a contracted facility for patients who need a higher level of skilled support to manage uncontrolled symptoms. This care is designed to be short-term. Patients requiring assistance with symptom management can be transferred to a facility that partners with IDT Hospice Care, Inc. for inpatient services.
